Epi Info looks like its a little better packaged but you are going to run into a lot of the same requirements. Though I will admit for usability on the endpoint, the person actually entering data, Epi Info looks easier. Also there are some decent deployment options for Microsoft Azure,(cloud).
With no team to manage any off these applications, a hosted solution will be your best bet. I'm saying this becuase security becomes the big concern when you start capturing PII. You want to make sure all the data is safe or if there is a breach, its not on you/(the company). I dont know a ton about this type of software, so i cant give you great recommendations.
alright so this is going to require a server, (a dedicated machine to run just this application and its required components). It runs a Database, PostgreSQL, and has a couple dependencies.

Hopefully this helps put some perspective on this project. If you have never done this kinda setup before, im not gonna lie, its gonna be ugly and messy. This install looks like it has a lot of room for error and, by the notes, really they expect someone to be maintaining it all the time.
If the company is looking for a "free" solution, this isn't it. The hardware costs and time requirements to maintain an application like this running at scale is going to out way the free aspect.
Gonna hit you with a bunch of questions. Are you looking for this? DHIS2 Downloads Was it already configured/setup or is this new? The github is just describing how to install DHIS2 on LXD, are you trying to accomplish this?
