You used the converter wrong though. A calculator like that will use the density of pure water (one gram per mililiter) to give you a value when you try to convert mass to volume, but ng/mL isn't a conversion it's a concentration. It means that in each mililiter of his blood, there were eleven nanograms (.011 micrograms) of fentanyl.
The overdose risk chart you posted is based on the amount consumed, not the concentration of what makes its way into the bloodstream.
It wasn't an accident.
