Running gels for every test would be prohibitively expensive. You'd need time and somewhat trained personnel for that kind of operation. You could use the Agilent micro-fluidics gels, but that's even more expensive and needs even better-trained techs. That's the problem--they're probably using some qPCR-like chemistry (probably SYBR-Green) and measuring fluorescence in a "dark-box" qPCR machine, and that cannot pick apart non-specific signal. The "garbage" signal glows the same as the "real" signal, so this is where the cycle thresholds matter. The amplification is not linear, so once you get to a certain point, you're exponentially amplifying garbage signal and just reading the fluorescence intensity. Completely pants on head retarded. If you tried to pass this bullshit off in a [good] research lab, your PI would put a foot in your ass at lab meeting. AND REMEMBER--THEY'VE DESTOYED LIVES WITH THIS SHIT SCIENCE.

It's fine to do 40 cycles if you have a refined protocol with optimized primers and you are looking for a binary result. You'll just see a lot more non-specific shit at the bottom of the gel.
With these tests, like you said, you can amplify crap from a dirty sample that's not necessarily "the virus," and who knows what thresholds they have been using.
